We built our own triage layer to classify them into actionable tickets, digests and exceptions, combining our IT and security experience with software and automation. Where AI-Built Software Can Go Wrong

These are the failure modes we check for in every review — real ones we see regularly, not theoretical risks. Knowing exactly where to look is most of the job.

! Code you can't explain. If you don't understand what the AI built, you can't maintain it or spot when it's creating hidden risk.
! Scope creep by stealth. The AI "fixes" a problem by rewriting half the application — a fundamental change you didn't ask for.
! Tests that prove nothing. The AI writes tests that pass because it also weakened the test criteria. The problem is hidden, not solved.
! Hallucinated dependencies. The AI references APIs, libraries or database tables that don't exist. The code looks right. It isn't.
! Security blind spots. API keys in code, input validation skipped, authentication that looks right but isn't. The AI optimises for "works", not "secure."
! The compulsive loop. You keep prompting, the AI keeps changing things, and the codebase gets more fragile with every iteration. Knowing when to stop is a skill the AI can't teach.
